CANCELLATION/RESCHEDULING POLICY:

At Supreme Wellness and Aesthetics, we value your patronage and the trust you place in us. To ensure
that we provide the highest quality service to all our clients, we kindly request that you provide us with a
24-hour notice if you need to cancel or reschedule your appointment. Please note that failure to provide
adequate notice or no-shows may result in a charge of $50 to the credit card on file.

CONSULTATIONS:

We offer complimentary consultations for all our aesthetic services. For wellness services, a $150
consultation fee applies when performed by a Registered Nurse or Wellness Coach, and $350 when
performed by a doctor. During the consultation, a customized treatment plan and all further fees will be
explained. Please note that these fees are not covered by insurance. Established clients will receive a
complimentary consultation each time they plan to receive a new service.

YOUR APPOINTMENT:

We follow standard operating procedures by taking photographs of our patients before and after
treatments. Your privacy is our priority, and the photographs will not be published or shared with anyone
without first obtaining your expressed written consent. Additionally, we ensure that we will not discuss
client treatments with any other clients. We kindly remind you that full payments for all procedures are
due at the time of treatment.

PAYMENTS:

To make your experience with us more convenient, we provide a range of payment options that may be
used individually or combined according to your preferences. We accept cash, gift cards (available for
purchase in any denomination), and major credit cards, including Visa, Mastercard, and Discover. We
also offer payment services through Care Credit. Please note that gift cards are non-refundable. All pre
purchases must be used within 1 year from the date of purchase.

REFUND POLICY:

At Supreme Wellness and Aesthetics, we offer services that are irrevocable. Therefore, we do not issue
refunds for any product or service that has been injected or used in your treatment. If a package or
series of treatments has begun and you wish to discontinue your treatment, we may extend a credit for
the prorated share of unused treatments at the discounted package price. This credit may be used to
purchase other treatments or products offered by Supreme, excluding Botox and fillers. Please note that
compounding pharmacies do not accept returns of products formulated specifically for your individual
needs.

SERVICES:

We offer elective procedures, and while we accept the risks and complications of the procedure, we do
not imply any guarantees as to the outcome. We encourage you to read specific treatment consents
carefully to make informed decisions.

HIPAA POLICY:

We are committed to maintaining the privacy of protected health information as defined by HIPAA. We
follow all the legal duties and privacy practices regarding health information about you, and we will
communicate with you by email, cell phone texting, or calls as necessary. Additionally, we will notify you
of any breach of protected information as required by federal and state law.

PROTECTED HEALTH INFORMATION (“PHI”):

Protected health information can be verbal, written, or electronic, and we take great care to ensure its
privacy and confidentiality.

We use and disclose Health Information for your treatment and to provide you with treatment-related
health care services. We may also use and disclose Health Information for billing and payment purposes,
appointment reminders, and to provide you with information about treatment alternatives or
health-related benefits and services. When appropriate, we may share Health Information with a person
involved in your medical care or payment for your care, such as your family or a close friend. In
emergency situations, we may notify your emergency contact of choice about your location or general
condition.

There are certain circumstances where we may need to disclose your Protected Health Information (PHI)
to others, but we always aim to respect your privacy and give you an opportunity to object or opt out.
For example, we may disclose your PHI to a family member, relative, or close friend who is involved in
your care or payment for your care, unless you object. If you are unable to object, we may disclose such
information if we determine that it is in your best medical interest.

However, for other uses and disclosures of your PHI, we require your written authorization. This includes
uses and disclosures for marketing purposes, as well as any other uses not covered by this notice or the
applicable laws. If you do provide us with authorization, you may revoke it at any time in writing, but any
disclosures made in reliance on your authorization before you revoked it will not be affected.

You also have the right to request that we communicate with you about medical matters in a specific way
or location, such as by mail or at work. We will try to accommodate your request as much as possible.
Finally, you have the right to a paper copy of this notice, which you may request at any time. Please note
that we reserve the right to update this notice to comply with any changes made to the HIPAA Notice of
Privacy Practices.

TERM OF AGREEMENT:

This agreement shall commence on the Effective Date and shall continue until canceled or modified by
agreement of the parties.

ENTIRE AGREEMENT:

This agreement constitutes the entire agreement between the parties with respect to the subject matter
hereof and supersedes all prior and contemporaneous oral or written agreements, understandings and
representations.

TREATMENT COMPLICATIONS

In the event of complication post-filler treatment, the correction would be provided at no cost. In the
event of an allergic reaction or sensitivity to the hyaluronic acid filler, hylenex will be provided at no cost.
Re-injection will not be provided as part of the treatment plan; the recommendation is not to introduce
the pathogen.

COUNTERPARTS:

This Agreement may be executed in counterparts, each of which shall be deemed an original, but all of
which together shall constitute one and the same instrument.

SEVERABILITY:

If any provision of this Agreement is held to be invalid or unenforceable by a court of competent
jurisdiction, such provision shall be severed from this Agreement, and the remaining provisions shall
remain in full force and effect.

AMENDMENTS:

No amendment, modification or waiver of any provision of this Agreement shall be effective unless in
writing and signed by both parties.

COMP SERVICES:

By accepting comp services you are forfeiting your rights to post negative reviews on any platform or
social media.

MISCELLANEOUS

Any services purchased including packages are valid for one year. If unused, they will expire 12 months
after the date they were purchased
